Job Summary
Under general professional supervision, performs social work/case management services including planning, organizing, treatment and inpatient discharge/disposition. Delivers social services in accordance with the approved plan of treatment and plan of care. Assists the physician and other healthcare members in understanding significant social and emotional factors related to the patient’s health problems.

Ability to collect key information from the patient, family and the community as part of the social work assessment.
Ability to communicate to the appropriate staff information about the patient’s discharge plan. Certificate/License/Registration: None. All Certifications and Licenses required for this job must be kept current as a condition of continued employment.
The social worker will:

- Use a high level of skill in assessing and treating complex psychosocial needs of citizens and their family members/caregivers often during times when people are experiencing crisis

- Demonstrate knowledge of a broad range of psychosocial, medical, and mental health issues
- Provide group and individual counseling appropriate to level of licensure
- Coordinate care, develop and use community resources to facilitate referrals, develop and implement treatment plans and provide accurate information to citizens, family, staff, and other internal and external customers in a timely manner
- Work closely with an interdisciplinary team. It is important that the incumbent is a team player
- Provide coverage as needed for co-workers, which may include social work coverage in other parts of the System
- Comply with all policies/regulations of the organism
- Attend and participate in administrative and clinical staff meetings
- Participate in training program, as well as, serve on facility, committees and workgroups.
